Dr. Mehmet Oz has been a medical mentor to Charlie Sheen since the actor went public with his HIV diagnosis — even getting Sheen to kick booze for yoga — and now, Sheen will appear on “The Dr. Oz Show” on Monday, Page Six has exclusively learned.

Sheen wants to “show how he’s turning over a new leaf,” said an insider. We hear Oz first approached the star about appearing after Sheen went on “Today” in November, but Sheen declined, insisting he was only going to talk to Matt Lauer, and instead sent his doctor and a former girlfriend to appear.

But insiders tell us, “Oz still met Sheen at his hotel that night . . . [They] spoke about alternative ways to better his health in addition to taking his medication. And they also had a serious heart-to-heart about other lifestyle changes.” The source added, “Oz is a real doctor first before a TV host, and [Sheen] feels comfortable opening up to him. Oz was like, ‘I get it. You’re an addict, but you have to cut all those behaviors because drinking weakens your immune system.’ He agreed he would stop drinking and start exercising again.”

Sheen had previously appeared on Oz’s show to seek help with his addictions after losing his job on “Two and a Half Men.”

“They have a history together. Oz was trying to convince him to stop smoking by showing him a smoker’s lung and a regular lung,” said a source.

Sheen agreed to do the new interview in part to show Oz he’s stuck to his promise after their hotel meeting.

For the interview, the duo also spent hours together at Sheen’s home. “He wanted to show . . . his new daily routine, [which consists of] jogging and yoga,” said the source. “He quit drinking right after he did the Lauer interview.”

In a promo for Oz, Sheen greets the doc at his front door with, “Well, good morning! Why don’t we go run?”

Sheen, 50, later shares an eerie dream he had at 28, when, “I had a sign around my neck that said, ‘AIDS.’” He tells Oz, “It’s going to be the most difficult thing I’ve tried to tackle,” but “this disease chose the wrong person.” An insider said, “He feels like this happened for a reason.”
